1. This rebate offer is available to qualifying end-user purchasers of a qualifying product. If any terms and conditions are not met the rebate will be denied.
2. Your purchase must be from a CARL ZEISS SPORTS OPTICS Authorized Dealer in the US.
3. Offer is valid for consumer purchases only on new and current specified ZEISS product, purchased between 11/01/2009 and 12/31/2009. Carl Zeiss distributors and dealers may not participate in this offer. Visit zeiss.com/sports for a complete listing of qualifying products and item numbers that are included in this promotion.
4. As proof of your purchase, and in order for your rebate request to be processed, in addition to the fully completed online registration or rebate card, you must submit a legible photocopy of the dated sales receipt and the original UPC code (cut from the side panel of the product package). Photocopies of UPCs are not accepted.
5. Your rebate request form must be postmarked on or before that date which is thirty (30) days from the date of your purchase. Any requests postmarked or received after applicable dates will be denied.
6. Rebate is not available on items returned. By submitting for this rebate, you affirm that you have not returned your item. Items with the original UPC label removed are not eligible for return.
7. CARL ZEISS is not responsible for lost, late, mutilated, misdirected or postage due rebate card request submissions. Illegible, fraudulent and incomplete rebate card request submissions will be considered invalid and ineligible, and will not be processed.
8. Fraudulent submission of multiple requests could result in federal prosecution, imprisonment, and/or fine under the U.S. Mail fraud statutes (18 USC Section 1341 and 1342).
9. Payment will be made by Carl Zeiss VisaŽ Prepaid Card. Cards are issued by Citibank, N.A. pursuant to a license from Visa U.S.A. Inc. and managed by Citi Prepaid Services. This card can be used everywhere Visa debit cards are accepted. Cards will not have cash access and cannot be used at ATMs. You can receive cash for the amount of your VisaŽ Prepaid Card balance from any VisaŽ member bank branch for no fee, just look for branches displaying the Visa logo. The Prepaid VisaŽ Card will expire 12 months from the issuance date. Your Carl Zeiss VisaŽ Prepaid Card will be mailed within 8-10 weeks from the date your completed rebate card request form and accompanying materials are received.
10. Carl Zeiss VisaŽ Prepaid Card will be issued in US-Dollars and mailed to addresses in the U.S. only.
11. Rebates may not be combined or used with any other rebate, free, or other promotional offers.
12. Rebate materials received become the property of CARL ZEISS and will be neither acknowledged nor returned.
13. This Rebate offer is valid only in the U.S., and is void where prohibited, taxed or otherwise restricted by law. To be eligible for the rebate, the individual submitting the rebate card request form must be a legal U.S. Resident who is at least 18 years of age at the time of submission.
14. Offer excludes items won or purchased at auctions, fundraising banquets or similar events. Offer excludes sample products, used products and returned/exchanged products.

16. Please remember to keep a photocopy of all materials comprising your rebate request submission.
17. Rebate application status updates, approval, denial and other notices may be sent via e-mail. You may check the status of your rebate by visiting the link provided in your email or visiting zeiss.com/sports. Please allow 3 weeks after mailing to make an inquiry regarding your rebate. |
