1. Your purchase must be from a CARL ZEISS Authorized Dealer.

2. As proof of your purchase, and in order for your rebate request to be processed, in addition to the fully completed rebate card, you must include a legible photocopy of the dated sales receipt.

3. Your rebate request form must be postmarked on or before that date which is twenty (20) days from the date of your purchase.

4. Rebate is not available on items returned. By submitting for this rebate, you affirm that you have not returned your item.

5. CARL ZEISS is not responsible for lost, late, mutilated, misdirected or postage due rebate card request submissions. Illegible, fraudulent and incomplete rebate card request submissions will be considered invalid and ineligible, and will not be processed.

6. Fraudulent submission of multiple requests could result in federal prosecution under the U.S. Mail fraud statutes (18 USC Section 1341 and 1342).

7. Rebate checks will be mailed within six (6) weeks from the date your completed rebate card request form and accompanying materials are received.

8. Rebate checks will be issued in US-Dollars and mailed to addresses in the U.S. only. Please remember to cash your check promptly.

9. Rebates may not be combined or used with any other rebate, free, or other promotional offers.

10. Rebate materials received become the property of CARL ZEISS and will be neither acknowledged nor returned.

11. This Rebate offer is valid only in the U.S., and is void where prohibited, taxed or otherwise restricted by law. To be eligible for the rebate, the individual submitting the rebate card request form must be a legal U.S. Resident who is at least 18 years of age at the time of submission.

12. CARL ZEISS reserves the right to cancel, rescind or extend this offer at any time due to business conditions or circumstances beyond its control.
