A step towards flapless, minimally invasive laser correction

ReLEx® combines state-of-the-art femtosecond technology with high-precision lenticule extraction aiming at providing minimally invasive refractive correction in a single system: the ZEISS VisuMax®. A refractive lenticule is created in the intact cornea and removed via a small incision. No ablation. No flap.

Flapless

ReLEx is turning the world of refractive surgery on its head. Previously a side-cut of roughly 20 mm had to be created, whereas a small incision of less than 4 mm is sufficient in ReLEx smile.

Single-step
ReLEx brings together a number of factors which clearly belong together: the entire laser treatment, consisting of lenticule creation and access, takes place in a single step.

Treatment Steps

Refractive correction with Femto-LASIK involves first cutting a flap and folding it back in order to remove corneal tissue using point-by-point-ablation technique. Both the flap itself and the excimer laser ablation process have been replaced. The lenticule extraction and the flapless nature of ReLEx® smile yield a large number of benefits.

Step 1:
In a single step, the VisuMax creates a refractive lenticule and a small incision of less than 4 mm in the intact cornea – virtually regardless of the ambient conditions or corneal structure.

Step 2:
The lenticule is removed through the small incision. The disruption to the biomechanics of the cornea is minimal. No flap is cut.

The removal of the lenticule changes the form of the cornea, creating the required change to the refraction.

Step 1:
In a single step, the VisuMax creates the lenticule and the flap-like access incision in the otherwise intact cornea. You, the surgeon, benefit from the smaller incision diameter in comparison to that of a LASIK procedure.

The flap-like access incision is opened and folded back.

The lenticule is removed manually via the flap-like access point.

The upper corneal layer is now flipped back. The changed form of the cornea corrects the refraction error.

Minimally invasive – ReLEx smile in comparison to LASIK

ReLEx® is turning the world of refractive surgery on its head. With ReLEx smile, one small incision - smaller than the earlier flap hinge - is enough to remove the refractive lenticule. For the first time ReLEx therefore permits minimally invasive refractive correction.

Side-cut up to 80% shorter
Previously a side-cut of roughly 20 mm had to be created, whereas a small incision of less than 4 mm is now sufficient. This means that the majority of the upper corneal layers remain untouched.

Cap incision area up to 30% smaller
The lamellar incision area in the cornea is roughly 1/3 smaller than that in Femto-LASIK.

Greater integrity of the upper corneal layers
The upper corneal layers remain practically untouched. It is therefore logical that the biomechanical stability of the cornea will be largely unaffected.

Less severe dry-eye syndrome
The minimally invasive incision gives rise to fewer transected nerves in the corneal surface and therefore to significantly reduced severity of dry-eye syndrome.

No flap-related complications
A minimally invasive incision can further reduce the incidence of infections, epithelial ingrowth or flap complications - and help the epithelium to heal much more quickly.

ReLEx makes exclusive use of the ZEISS VisuMax - giving you access to the benefits of high-precision femtosecond technology: high level reproducibility and predictability - even in the case of major corrections.

Precise lenticule extraction instead of point-by-point ablation
Lenticule extraction replaces point-by-point excimer laser ablation. In comparison to excimer laser-based procedures, this gives you, the surgeon, extra time and a high degree of predictability—especially when correcting higher refraction values.

Reproducible results
ReLEx is not dependent upon complex nomograms or fluence tests. Intraoperative ambient conditions or individual corneal characteristics have virtually no influence on the reproducibility of the femtosecond lenticule incision.

Outstanding predictability
Post-treatment examinations show that: the VisuMax creates the pre-calculated lenticule incision with utmost precision and predictability in the cornea. The spread of results is very small and is virtually unrelated to the degree of refraction correction.

Saves time and improves patients comfort

ReLEx brings together a number of factors which clearly belong together: the entire laser treatment, consisting of lenticule creation and access, takes place in a single step. The change to the excimer laser necessary in LASIK and Femto-LASIK procedures is not required in ReLEx, bringing a wide range of benefits.

No patient relocation
The entire refractive correction is undertaken using a single system, the VisuMax, meaning that the patient does not need to be relocated. Unlike in Femto-LASIK, only one treatment planning procedure is therefore necessary.

Short procedure
Only slightly more time is required to generate the lenticule than for a simple flap cut—and removing the lenticule takes hardly any longer than opening the flap.

Maximum protection for cornea
Since the lenticule and access are created together, no tolerances must be maintained for further treatment steps, and the largest possible portion of the cornea remains untouched.

The clinical study reports on the results of the ReLEx® smile procedure for the correction of myopia and astigmatism (3 month results).*

Background and method

  • 269 eyes were analyzed and evaluated in three study centers.
  • All the eyes had a BCVA of 20/20 or better before the operation.

Predictability

  • Consistent results with outstanding predictability
  • Very close to target refraction
  • Valid across entire range of correction, from low to high-level myopia

Efficacy

  • 85% of all patients with a preoperative best-corrected visual acuity of 20/20 or higher achieved uncorrected visual acuity of 20/20 or higher after surgery.

Stability

  • Impressive stability of the refractive correction (MR SEQ)

Precision

  • 100% of all patients achieved target refraction within the typical "satisfaction zone" (± 1 D)
  • 97% of all patients achieved target refraction with a precision of ± 0.5 D
