Removing hard cataracts can be difficult even for the most experienced cataract surgeons. Patients are typically older, with low visual acuity and coexisting ocular pathologies. Thus, it is essential to understand the patient's underlying conditions and to prepare for possible challenges during the surgical procedure.

On the one hand, pre-operative cataract evaluation is an occurring challenge since a dense cataract is, by its very nature, difficult to penetrate. On the other hand, lens fragmentation of a dense cataract requires a lot more phaco energy to emulsify the hard nucleus. More energy increases the risk of damage to corneal endothelial cells, which, in turn, can cause post-operative complications.

Dr. Don Pek

Tan Tock Seng Hospital, Singapore

Dr. Don Pek is Senior Consultant and Assistant Head of Anterior Segment and Cataract Service at Tan Tock Seng Hospital, Singapore and Assistant Director of the Ang Mo Kio Specialist Centre, Singapore.

  • He received his medical training from the University of Glasgow and also completed a Bachelor of Medical Science Honours degree. He completed his post graduate training and has been elected as a Fellow of both the Royal College of Surgeons of Edinburgh and a Fellow of the Academy of Medicine, Singapore. Moreover, he did a fellowship for Anterior segment surgery, trained with George Beiko, St Catherines, Ontario.
  • Dr. Pek has special interest in refractive surgery, complex cataracts and anterior segment reconstruction surgery. He is an invited trainer for ASCRS scleral fixation wetlab. Furthermore, he is regularly involved in providing cataract surgery to people with poor access to healthcare in regions such as Myanmar, Cambodia and Ladakh.
Dr. Ruth Lapid-Gortzak

Amsterdam University Medical Centers, Amsterdam, Netherlands

Dr. Ruth Lapid-Gortzak is a Cataract, Cornea, and Refractive Surgeon at the Amsterdam University Medical Centers (University of Amsterdam) as well as in private practice (www.retina.nl).

  • She is an expert in multifocal IOL implantation, complicated cataract surgery, refractive surgery, and does academic research in cataract and refractive surgery outcome studies, and straylight studies in refractive surgery. In addition, Ruth Lapid acts as an ophthalmic device development expert with the ophthalmic device industries and start-ups.
  • Dr. Lapid graduated from the Hadassah and Hebrew University Medical School in Jerusalem, winning an award for her MD thesis. She did her residency in ophthalmology at the Soroka University Medical Center in Beer Sheba. A fellowship in Cornea, and external diseases with Prof Rootman and Slomovic followed at the University of Toronto in Canada. 
  • Dr. Lapid obtained her PhD-degree in “Visual quality improvements in refractive surgery” at the University of Amsterdam. In 2018 she was granted membership of the International Intraocular Implant Club (IIIC). She is a member of the Board of the Netherlands’ Intra Ocular Implant Club since 2013.

Dr. Nino D. Hirnschall

Hanusch Hospital, Vienna, Austria

Associate Professor Nino Hirnschall is a Consultant ophthalmologist at Hanusch Hospital, with high expertise in biometry and cataract surgery.

  • Additionally, in 2018 he absolved a surgical cornea fellowship at Sydney Eye Hospital, Australia. Among others, he was awarded as “Top 40 under 40 – most influential ophthalmologist worldwide” in 2015 and he won the “Peter Barry Fellowship Grand” in 2018.
  • In the past years, Assoc. Prof. Hirnschall has given 125 talks at international conferences and meetings and published more than 85 articles and book chapters. He is reviewer for several renowned journals.
Dr. Sartaj Grewal

Grewal Eye Insitute, Chandigarh, India

Dr. Sartaj Grewal is consultant for Cataract and Refractive Services in Grewal Eye Insitute, Chandigarh.
  • In 2017, Dr. Sartaj completed his MD in Ophthalmology from RP Centre of AIIMS, New Delhi.
  • Among others, Dr. Sartaj is member of the American Academy of Ophthalmology, the All India Ophthalmological Society and the Delhi Ophthalmological Society.
  • He is author of numerous publications and has given several presentations at national and international conferences. In the past years, he has served as a Co-Instructor for courses at the All India Ophthalmological Society Annual Meetings.
  • In 2017 and 2018, Dr. Sartaj has received the Best of Show Video award by the American Academy of Ophthalmology.
Dr. Franco Spedale

ASST Franciacorta Chiari, Brescia, Italy

Dr. Franco Spedale is the Head of Ophthalmology at ASST Franciacorta Chiari, Brescia. As anterior segment expert, he practices interventions for the correction of the main visual defects and presbyopia. 

  • He has performed more than 15,000 cataract surgeries using the most innovative techniques and vitreoretinal interventions for macular pucker and lamellar keratoplasty.
  • Dr. Spedale is member of various authoritative scientific societies and he has been a speaker at several national and international congresses. 
  • He has performed numerous observational studies for major pharmaceutical companies to test new products to be placed on the market.
  • He graduated in Medicine and Surgery from the Faculty of Medicine and Surgery of the University of Brescia with full marks in 1995, in he obtained a specialty diploma in Clinical Ophthalmology with the highest result. In May 2000 he began his professional career at the Operating Unit of the Desenzano del Garda hospital. He carried out his profession at the U.O. of Ophthalmology of Borgo Trento of the Instituti Spitalieri di Verona completing not only professional development, but also the management of patients, mainly elderly, of their problems and expectations connected to them. He directed the retinal diagnostic sector of the U.O. of Verona di Borgo Trento. In 2006, Dr. Spedale opened and directs the U.O. of Ophthalmology at the ASSt of Franciacorta.
