Abstract

Guided immediate implant placement with simultaneous soft and hard tissue grafting and immediate loading

This video demonstrates fully guided immediate implant placement in the maxillary aesthetic zone with simultaneous hard- and soft-tissue grafting and immediate loading under microscopic magnification. The precise approach is designed to preserve the existing anatomy as effectively as possible and create stable conditions for a natural, long-lasting aesthetic outcome through a biologically driven approach using autogenous hard- and soft-tissue grafts.

Following atraumatic extraction and thorough socket debridement, a surgical guide enables prosthetically driven three-dimensional implant positioning. Microscopic magnification facilitates the precise placement of particulate autogenous bone within the peri-implant gap and the atraumatic preparation of a facial tunnel for the accurate positioning of the connective tissue graft. It also enhances precision during graft harvesting and supports meticulous, complete extraoral de-epithelialization. Adequate primary stability permits immediate loading with a customized screw-retained provisional restoration. Its customized transmucosal contour stabilizes the blood clot and grafting materials, supports the papillae, and guides soft-tissue maturation.

Microscopic magnification and coaxial illumination improve visual control during extraction, debridement, graft placement, and suturing. They enable precise, atraumatic tissue handling, preservation of vascularity, and fine wound closure. Combining digital planning, guided surgery, autogenous tissue augmentation, and microscope-assisted techniques provides a minimally invasive and predictable approach to a natural, long-lasting aesthetic outcome.
