脳・神経系疾患の診断および研究
神経病理学は、脳や神経系の疾患の診断や研究を中心に行う病理学の一分野です。患者の治療や法医学的な理由で診断が求められる場合、顕微鏡や分子的手法を使って、神経外科的に切除された組織検体を解析します。神経病理学の対象は、膠芽細胞腫、神経変性疾患、様々な神経発達障害などの中枢神経系(CNS)の腫瘍や疾患です。
神経病理医は、脳腫瘍の患者の治療決定などにおいて重要な役割を果たします。なぜなら、神経病理解析の結果が診断の要になるためです。診断確定後に、キャンサーボードで神経外科医や神経腫瘍医が話し合い、患者の治療方針を決定します。
神経外科では、術中の意思決定に造影剤を使ったイメージング技術が使われるケースが増えています。1よくみられるのは蛍光領域を可視化するシステムで、適切なフィルターと光源を搭載した術中顕微鏡が使われます。1この技術を応用した共焦点内視鏡(CEM)2では、高倍率で組織の微細構造を可視化できます。これにより、リアルタイムかつin vivoでの(組織を摘出しない)イメージングが可能になります。1CEMでは蛍光色素を使い、硬性内視鏡のようなスキャンプローブを組織の表面に接触させるだけで、細胞内の構造を確認することができます。1
